This is completed once for the life of services with UniCultural Group Services. The intake packet should be returned prior to scheduling an appointment. This fee is per person NOT PER FAMILY unit. For family counseling, please contact our office for pricing. No refunds are given 72 hours prior to the event, however, a credit will be given for future use.
